I agree that I fully own all device(s) and have removed any iCloud Activation Locks, Firmware Passwords, and/or removed my device(s) from a business Remote Management profile. Device(s) have been paid for in full and are not part of any installment plan or on the cellular blacklist.

I agree to pack my device in accordance with the included shipping instructions, and understand failure to do so may result in damage that is NOT covered by shipping insurance

Once submitted devices cannot be returned under any circumstances. Once the devices are received and inspected, the donor will receive a tax-deductible in-kind donation receipt from the Miracle Children's Foundation. If the devices are unable to be used because they do not match the information provided, I understand the devices will not be returned and will be safely e-disposed. Miracle Children's Foundation can not send out in-kind donation receipts for e-disposed devices.
